In loving memory of Derek Addy who sadly passed away on 13th March 2017 at Dewsbury District Hospital, aged 84 years.

Funeral service will take place at Dewsbury Moor Crematorium on Friday 31st March 2017 at 2pm.

Family flowers only please but donations in memory of Derek may be placed in the collection box provided for the benefit of Alzheimer's Research.
